Aims of the project

Hypotheses:

1. Hyperbaric oxygen (HBO) and/or Aminoguanidine (AMG) reduce osteonecrosis in idiopathic osteonecrosis of rat femoral heads.

2. HBO and AMG will act to reduce osteonecrosis by reducing osteoblast apoptosis and expression of inducible nitric oxide synthase (iNOS), as well as improving osteogenesis of bone marrow cells.

Specific Aims:

1. To determine if HBO and/or AMG prevent idiopathic osteonecrosis in a rat model

2. To elicit mechanisms of HBO and AMG action in osteonecrosis by investigating

a) osteocyte apoptosis in an established model of osteonecrosis

b) expression of eNOS and iNOS in an established model of osteonecrosis

c) osteoblastogenesis and osteoclastogenesis of bone marrow cells derived from osteonecrotic rats

Expected outcomes and significance

This research will provide sound scientific evidence on whether or not HBO and/or AMG are beneficial as a treatment of idiopathic osteonecrosis. Additionally, this research will determine if these treatments influence osteocyte apoptosis and bone-forming ability of bone marrow cells. If positive effects are found in the rat model, it could form a basis for clinical trials of HBO and AMG for treatment of non-traumatic idiopathic human osteonecrosis.
